I actually don't know the song title, but I found out that Janet Tamaro herself with someone else [I forgot the name] wrote is especially for this episode.

The song is "I'm Still Here" da James Levine. It was co-written with Janet Tamaro, the full song has not been released yet. It's rumored to release sometime this year. There is a short version that is on Youtube that te can download :D
