How do te get a dedicated medal in a fan club?

I've seen people with them. I want to get one for the Demi Lovato club. So how do te get a dedicated?
(I want to get a fanatic but that seems hard so I'll start with dedicated)

But quite honestly it's all random, I actually received a medal on the Natale spot in June (I hadn't been active there in months).
